Instructions
*Please contact one of our locations first before sending any devices for repairs or unlocks.
*Complete this form and print it out to attach with your device.
*Send your device with battery.
*Do not send sim card, cash, or checks.
*Our technician will call you once we recieved and troubleshoot your device.
*Payment will be taken over the phone or paypal. We do not accept checks or cash for mail-in services.
*Diagnostic fee ($5) + shipping charge will be charged if your device cannot be repaired or refused repairs.
